To obtain an incident report you can do any of the following:

1. Submit an online request via CWV311
2. Call 304-348-6400
3. Visit the Records Division at City Hall, located 501 Virginia St. East (Court St. Side of building) in person with photo ID.

You will be notified when your report is ready to be picked up at the CPD Records Division at City Hall - 501 Virginia St. East (Court St. Side of building).

There is a $10 Charge. Only cash or check accepted.

Please bring a photo ID.

The person must be named as a victim in the report in order to obtain a copy.

Only basic information is provided.

Additional report information is only available via a subpoena or discovery request through the City Attorney's Office.
