• Class or Program Proposal (Type of class, age group, etc) 
  
• Relevant Experience & Qualifications (certifications, past teaching experience, or expertise) 
  
• Availability (preferred days/times) 
  
  In the event, we do not have any instructor opportunities available, we will keep your information on file for 1 year.
